Notice is hereby given that Leoncia A. ballesteros, with office address at Units 9, 10, 13, and 14 Metrococo Export Corp. Building 1B, 105 Industry Road Phase 1, Laguna Technopark, Brgy. Don Jose, Sta. Rosa City, Laguna had executed an “Affidavit of Loss” before notary public Atty. Benedicto M. Jose as per Doc. No. 522; Page No. 106; Book No. 8; Series of 2023, stating among others, that:
• she is the Finance and Accounting Manager of Furukawa Electric Thermal Management Solutions and Products Laguna Inc. which is the surviving entity after the merging with Lemtech Philippine Thermal System Inc.;
• the official receipt of the Meralco deposit of Lemtech Philippine Thermal System Inc. in the amount of Php2,380,550.00 was lost and cannot be found despite diligent efforts to locate it; and
• the previous accounting officer of Lemtech Philippine Thermal System Inc. who was the custodian of said document is no longer connected with the company.
